By my signature, I and or the company I represent accept the scope of work and payment terms. I understand that any unexpected changes in the scope of work may incur additional fees and charges (sub-base abnormalities such as mud/clay, pavement removal >3.5", unforeseen structures, tree roots, etc.). Any such occurrence will be discussed and agreed to at the time of discovery.

Parisi Paving, Inc will not be held liable for any unmarked utilities, low voltage wiring, previously damaged or weak perimeter pavements including city sidewalks or curbing, during the normal course of work. Any such occurrence will be responsibility of property owner to repair. Payment for work is due in full at the time of work completion.

A 5% service fee will be added to any open balance beyond 5 calendar days from work completion. A 1.8% fee will be applied to any remaining balance compounded for every 30 day period until payment is made in full. A 4% convenience fee will be added to all credit card payments. A $25.00 Fee will be applied for any returned checks for any reason. Material pricing is valid for 30 days from proposal date. All workmanship is 100% guaranteed for 1 year from date of completion.
